A person centred support plan is developed based on the the individual needs of the young person which is reviewed monthly. The number of hours of support provided will vary according requirements with the aim to gradually reduce over time as they gain more confidence. The support team work flexibly around the young persons timetable and support sessions will be agreed upon by both parties. Sessions can take place at the young persons accommodation however staff are happy to travel where support is required at appointments, interviews or meetings.

Accomodation
Young people are provided accommodation in a self contained furnished property. They will be expected to sign a tenancy agreement which will clearly outline what is included in their package. Weekly health and safety checks will be done at the property to ensure that there are no hazards and all equipment is working correctly
Housing support will be provided and young people will be supported to attend appointments at the housing office, bid online and when the time comes, move on to fully independent living.
